CCDM provides three documents here on-line in PDF format which you can use to submit your application for membership. You can open the PDF documents, print them, and then submit them to CCDM at the address provided.

In filling out the membership materials PLEASE READ ALL OF THE INSTRUCTIONS ON THE PRACTITIONER MEMBER APPLICATION AND QUALIFICATIONS PAGES.

If you do not meet all of the requirements for Practitioner Membership, you can skip the Practitioner Membership Qualifications and fill out only the Application for General and Student Membership.
